This form is for tenants who have received a 30-/60-/90-day notice to quit (eviction notice) that has not yet expired and who have not yet been sued by their landlord. If you have not received an eviction notice or if you have already been sued (received a complaint and summons), please do NOT fill out this form. For other resources, see below:
